A 62-year-old woman has died from injuries suffered in the collision which followed a police pursuit near Stratford-upon-Avon.
She had been driving a white Ford Puma when it was involved in a collision with a black Audi on the A439 Warwick Road on Monday, August 24.
Her death is the second fatality linked to the incident. The 19-year-old man driving the Audi was pronounced dead at the scene.
Warwickshire Police said officers first attempted to stop the black Audi on Birmingham Road at around 5.23pm. The vehicle failed to stop and made off towards the A46 before travelling back towards Stratford along the A439 Warwick Road.
Police later confirmed the pursuit lasted around six minutes.
At around 5.44pm, the Audi was involved in a collision with the white Ford Puma on Warwick Road.
The 19-year-old Audi driver was pronounced dead at the scene by paramedics. The Ford driver, a woman in her 60s, was taken to hospital in a critical condition and has since died from her injuries.
A woman in her 20s who was travelling as a passenger in the Ford was treated for minor injuries.
Police also said another driver attended hospital for further assessment, although his injuries were not believed to be serious.
At an earlier stage of the investigation, officers said they believed two other cars had been damaged as a result of the collision, while several other motorists had reported damage linked to the pursuit.
Warwickshire Police made a mandatory referral to the Independent Office for Police Conduct because a death followed police contact. The IOPC declared an independent investigation and Warwickshire Police said it would fully co-operate.

Police also confirm a woman in her 20s in the Ford suffered minor injuries, another driver attended hospital, two other cars were believed to have been damaged in the collision and several motorists had reported damage linked to the pursuit.
